Currently, the best-known and most often used model is the
PRECEDE-PROCEED model. It is comprised of nine phases or steps. The
first five phases, which make up the PRECEDE portion of the model, are
used for diagnosis. The last four phases, the PROCEED portion, are for
implementation and evaluation (process, impact, and outcome) steps. The
approach in this model is to identify the desired outcome and work
backwards to determine the cause that would lead to this desired
outcome.

Subject:        Population health framework

We are looking for a population health framework for both planning and
integration of population health perspectives and strategies across a
regional health system (i.e. the whole ball of wax, from prevention and
primary care to tertiary and long-term care). In particular, we are
looking
for a framework that combines population health and health promotion.

We have the Bhatti and Hamilton Population Health Promotion model but
are
also interested in other models.

Any ideas?
