Dennis - this is part of the Canadian Heart Health Initiative database
(which is the largest cardiovascular risk factor database and set of
interventions available internationally).  It was the basis for the most
recent CHHI Network conference in Halifax in the fall of 1998.  Heart
and Stroke is the key sponsor for the conferences and their senior
science officer the plenary speaker for the conference that had as its
focus - the Determinants of Health.

There are actually two documents on the Inequalities of Heart Health
that have been published by Health Canada in the last 8 years or so.
