I thought I would answer to the whole list given my reply might be relevant to others who were not aware that Ontario CHCs have in the past (and likely still do - although not sure
given I am not involved to the extent I once was) published a yearly "Green Book" which describes briefly all CHC-initiated health promotion programs from across the province of
Ontario. I suspect this still exisits and would give you what you need (and more).
