Program Evaluator Required

 See the North Bay and District Health Unit’s website for a full description
of this request for proposal.

 The North Bay and District Health Unit requires the expertise of a program
evaluator to:

 Develop a 4-year project evaluation plan based on the approved four year
project plan for the Injury and Family Abuse Prevention program, using the
template provided by the Public Health Branch, and providing any other
details requested by the Ministry, on or before April 15, 2003.

 Deliverables

 The program evaluator will work with designated staff at the North Bay and
District Health Unit and relevant partners to develop a comprehensive 4-year
evaluation plan for utilization by staff responsible for the implementation
of the 4-year plan for Injury and Family Abuse Prevention.

 The program evaluator will:

 Attend relevant meetings in North Bay (minimum of 2 face to face meetings)
and interact with designated staff by telephone and email as required;

Develop a detailed program logic model based on the existing 4 year plan;

Develop a comprehensive evaluation plan for the full 4 years of the
initiative for all activities and outcome objectives;
Develop the tools and methods for monitoring and measuring relevant process
and outcome indicators;
Identify consistent methods for reporting and summarizing key evaluation
findings for the purpose of further planning and reporting to key
stakeholders, including the Ministry of Health and Long Term Care;
Provide training and orientation to program staff responsible for both the
implementation and evaluation of the programs;
Be available on a consultative basis to assist staff in the implementation
of the evaluation plan within the first year.
 Timeframe

 The program evaluator must begin immediately and meet the deadline of April
15, 2003.
