1. what type of utilization tools do you use in your specific setting.
2. Do you utilize any screening tools? If so, which ones
3. Tools for Risk Stratification?

If you have a brief description on these tools and the outcomes that you
have been able to achieve, that would also be most helpful. Once I get this
information from the various sources I will post and share the results for
the group.
