I worked on an "Active Living at Work" proposal for the Region of Peel.  It
includes a program definition, rationale, recommendations and action plan,
literature review, proposed budget, cost benefit analysis, liability
implications, focus group results, proposed activities and an inventory and
gap analysis.  Although it is Region of Peel specific it may be helpful.
Also, Nancy Dubois has been working on this and may be a great resource for
you.  If you would like a copy of this proposal, just email me directly and
