contacting the Health Communication Unit at the Centre for Health Promotion
at U of T as we are funded by the Ministry of Health as a provincial
resource centre to provide support in just this sort of thing.  In fact, I
have just finished working with one health department on just this topic.

planning framework - not the example you were looking for but perhaps a

     I have recently been assigned the task of developing a Chronic Disease
Plan for a local health department.  Can anyone refer me to previous or
