Can anyone recommend any interesting organizations in New York City related to the following?  I am going to be there from April 2-5 to do some site visits and some learning to assist in the development of our organization.

1. Health care organizations that have effectively linked with population groups that
are traditionally underserved (homeless, recent immigrants and refugees
etc.)
2. Organizations that deliver effective mobile services to the above and
other population groups.
3. Organizations that have integrated a community development, health promotion, determinants of health approach into their work at a corporate level as well as at a front line level.
4. I would like to meet with staff at a management level whose responsibility is for access and equity, community development, community relationss, research,
determinants  of health etc.
5. Organizations which partner effectively with hospitals and community
based organizations.
6. Organizations with innovative, effective models of community
accountability  (governance, community representation,  advisory structures
at a governance and operational level).
7. Immediate care facilities that focus on accessibility to diverse
communities.

Other areas of interest:

1. Effective models of patient advocacy.
2. Research (community based)
3. Equity and Diversity strategies.
4. Diversity in the staff complement, horizontally and vertically.
5. LGBT programs.
