I am helping to evaluate an inner city schools project to prevent Type 2
diabetes. Although we have limited resources for evaluation, we would like
to do a simple knowledge-attitudes-behavior survey of grade 5 students that
we can track over the three years of the project. Can anyone point me
towards samples or examples of such a tool that could be modified for this
project?

We would appreciate any suggestions.
