Health in Action is a website at www.health-in-action.org/
with many of those resources and information that you are looking for or
will at least point you in that direction. It is utimately 'on-line
access to health promotion and injury prevention information' with
databases of contacts, programs, research and resources. Under web
connections you will find many links categorized to assist you in the
type of information needed (includes health promotion journals with web
addresses). As well, under theme pages, there are many resources listed
for evaluation in the field.
