diabetes.
Although it doesn't include a needs assessment tool, the information may be
helpful to you as develop your needs assessment.  It's at

Subject: Diabetes Needs Assessment


Hi,
I'm looking for a needs assessment tool on diabetes awareness and health
behaviour.  Knowledge of risk factors, physical activity levels and eating
habits need to be addressed.  This will be administered to the general
public, aged 20-64.  If anyone has anything even remotely close that you
could send me, it would be greatly appreciated.
