I am pleased to provide you with a copy of Health Canada's Population Health
Template (and the abbreviated Template Tool).  The Template organizes and
consolidates current understandings of population health under eight "key
elements", and describes procedures and processes required to act on each
element.

The Population Health Template can be used in various ways, including: to
support policy and program development and implementation; for training and
education on population health; to assess congruence with population health
concepts; to aid in evaluation; and as a basis for further knowledge
development.

The Template is a work in progress. I hope you will encourage the review and use
of this document within your organization and in so doing that you will provide
us with further insights to advance our collective thinking. If you would like
to provide a substantive review, please  contact me by return e-mail so I can
provide a short questionnaire to guide formulation of your comments.  Otherwise
general comments or suggestions are welcome.

J'ai le plaisir de vous envoyer le Modèle de promotion de la santé de la
population de Santé Canada et de l'Outil d'application du modèle.  Le Modèle
organise et regroupe les connaissances actuelles en santé de la population sous
huit « éléments clés » et décrit les processus et les procédures nécessaires
pour intervenir par rapport à chacun des éléments.

Le Modèle peut servir à plusieurs fins : appuyer l'élaboration et la mise en
oeuvre de politiques et de programmes; former et éduquer dans le domaine de la
santé de la population; vérifier la conformité aux concepts liés à la santé de
la population; aider à évaluer; orienter le développement des connaissances.

Le Modèle est une oeuvre en cours. J'espère que vous encouragerez l'examen et
l'utilisation de ce document dans vos organisations et que vous nous aiderez,
par vos observations éclairantes, à enrichir notre  réflexion collective. Si
vous désirez soumettre une analyse de fond, veuillez communiquer avec moi en me
retournant un message électronique afin que je puisse vous fournir un bref
questionnaire qui vous aidera à formuler vos commentaires.
