Hello
Capital Health, the district health authority for Halifax, NS and surrounding communities, is about to create a non-smoking policy for our facilities and programs.  The policy will also address the support and cessation needs of staff and patients/clients.  The organization employs approximately 10,000 people, has tertiary, secondary and primary care facilities, public health, addiction services, long term care facilites and rehab services.  

Before we go too far, we would like to survey staff re their tobacco use and readiness to quit, preferred cessation support, and also their support for implementation of the district policy.  Could anyone offer advice or a survey tool that could help us?  We have been in touch with Calgary DHA, but we're wondering if anyone else has experience to share.
