Hi Dennis,

I work as the Assoc. Director of Population Health for a medium size
health service in Sydney, Australia.

The health service is currently reviewing how our Population Health
Continuum fits in with our other governing areas Clinical Governance,
Corporate Governance and Community Governance. These four areas guide
all the work of the health service. Each has a governing committee which
reports directly to the Executive.

Clearly these are not discrete areas. All four service to inform each
other.

So my question, do you know of any other health authorities or services
which have adopted similar structures?

I'd greatly appreciate any contacts or pointers to relevant literature.
