Hi Ian:

Take a look at King County's program that ties actions taken to health
insurance co-payments,
maximums and deductibles.
• Bronze Level: Basic
• Silver Level: On-line wellness
assessment
• Gold Level: Complete resulting action plan
As of March 2006, 90% of
employees had registered
• Expected to save $18.5 million a
Year (I don't think they have yet published results. If you can find them
please let me know)

Also interesting is Michigan Hospital's Health Plus program
Incentives are based on an annual appraisal of
serum cholesterol, blood pressure,
tobacco use, body fat, physical
fitness, motor vehicle safety,
nutrition, alcohol consumption
There have been significant reductions in Medical Claims and Days Lost
