WE are trying to develop an evaluation for a health education intervention for children ages 8-12. I am wondering if there is anyone who could share some literature on developing measures for children this age.

Questions of concern are. 

How many response categories are acceptable and not too confusing (is a 5 point Likert ok or no?)

Has there been any theory validation done for children?

What are our options?
