Try  "The Health Bus" which visits 12 sites in downtown toronto and provides health care.

It was previously run by Wellesley hospital who did a needs assessment.

Hi,

We have a group interested in conducting an assessment to determine the
need for shelter housing and/or homeless programs in our community.  Can
anybody guide me towards needs assessment tools that may have been used
in other communities?
