REQUEST FOR PROPOSAL
SUDBURY / MANITOULIN DISTRICTS HEART HEALTH PROJECT
HEART HEALTH MANAGEMENT GROUP
VISIONING 2004-2008

DEADLINES
Proposal Deadline: February 28, 2003
Notification and Award of Contract: March 6, 2003

INTRODUCTION
Sudbury/Manitoulin Districts Heart Health Project is accountable to the
community through its guiding principles, mission statement, visioning and
work plans. It is accountable to the Ministry of Health through the Sudbury
& District Health Unit for fiscal responsibility and delegates the fiscal
management to the Heart Health Management Group.

Sudbury/Manitoulin Heart Health Project utilizes a comprehensive community
mobilization approach. This is achieved through four currently existing
geographic coalitions and a newly developing coalition, plus two advisory
committees - Workplace and Healthy Schools Healthy Kids. The project's
activities and resources address the risk factors contributing to heart
disease: physical inactivity, unhealthy eating/weights, tobacco use, and
stress.

The mission of the Sudbury/Manitoulin Districts Heart Health Project
involves
"Working together to promote Heart Healthy communities."

WHY VISIONING AT THIS TIME?
Renewed five year funding has been received for Heart Health Project across
Ontario. For this reason, we are seeking expertise from a consultant to
assist our project membership in a strategic visioning session for five year
planning. We need to review our current organizational structure, our
existing partnerships and the identification of possible gaps in
partnerships. We want to define the governance structure that best utilizes
resources to meet the mandate as well as ensure geographic inclusion.

CONSULTANT ROLE
1.       Review all pertinent documentation including current terms of
reference, list of current partnerships by coalition / working group, Heart
Health Management minutes, provincial evaluations, Continuation Working
Group documents and minutes from the Ontario Heart Health Program Phase II.
This is to be provided by the Heart Health Coordinator at the Sudbury &
District Health Unit.
2.       Dialogue as required with the Heart Health Coordinator and Heart
Health Management Chair.
3.       Facilitate Visioning Session on April 3-4, 2003 in City of Greater
Sudbury to meet the desired outcomes specified above.
4.       Submission of the final report by April 23, 2003 to the Heart
Health Management Group. The final report should also include proposed
structures and models of governance, recommendations, strengths and
limitations, needed partnerships, as well as include the input from
participants at the session.

BUDGET
Up to a maximum of $4,000.00 including the production and presentation of a
final report. Final report and working documents are to be provided on
diskette in MS Word format and electronically via email. Applicants should
consider this amount prior to submitting a proposal, as additional funding
is not available. An additional amount can be provided for travel expenses
of up to one thousand dollars ($1000.00). This will need to be submitted and
detailed in the proposal.

SUBMISSION OF FINAL MATERIALS
One copy (both hard copy, electronically via email and disk copy in MS Word)
of the final document must be provided. All materials generated during the
project are the property of Sudbury/Manitoulin Districts Heart Health
Project and the Sudbury & District Health Unit.

REPORTING RESPONSIBILITIES
The selected contractor is responsible to the Sudbury & District Health Unit
and the Heart Health Management Chair, through the project lead (Barb Eles)
of the Sudbury & District Health Unit, for the fulfillment of the work plan.


CONSULTANT SELECTION
Proposals will be accepted until February 28, 2003. Selection of the
contractor will be completed through the review of the submitted proposal,
which will be used to award this project.  Proposals received in full by
February 28, 2003 must contain the following:
1.        A one to two-page outline of the potential contractors'
understanding of the project.
2.        A work plan outlining the tasks to be undertaken, consultation and
revision. This work plan must be within the   stated time lines.
3.       Two relevant samples of authored material demonstrating technical
writing skills.
4.       Two references related to the work and scope of this project.
5.       CV or resume with summary of related achievements and
qualifications
6.       Technical support available to conduct business (e.g. fax and
email)

ASSESSMENT CRITERIA
The proposal, including all above-mentioned requirements, will be assessed
according to the adequacy, appropriateness and demonstrated ability and
understanding of the project.

PROPOSALS WILL BE EVALUATED ON:
1.       The appropriateness of the work plan.
2.       Experience and knowledge of the contractor.
3.       Experience and knowledge of northern and rural issues, working
across large geographic distances, and working within multicultural milieu
(Francophone and Aboriginal).
4.       The content of the one page outline including the contractors
understanding of the project.
5.       The detailed work plan as it relates to the project scope and time
lines.
6.       The ability of the contractor to work within the timelines.

The Sudbury & District Health Unit reserves the rights to refuse any and all
proposals; cancel and/or re-issue this RFP; and enter into negotiations with
one of more bidders.

SUBMISSION OF PROPOSALS
