 The issue surely is a sense of loss of control over the situation. It
really doesn't matter whether you are rich or poor. We have to empower
people to make decisions about their health which includes whether they
choose to follow the 'good health rules' or not. I agree it's usually much
easier to make and follow good health decisions if you are financially OK,
well educated and somewhat obsessive but it's not necessarily so. In some
cases changes of policy and social conditions will make a huge difference,
but even in extremely adverse circumstances some people and communities rise
above their dire situation. There is an X factor that we have yet to
describe or understand fully, but personal, family and community empowerment
(i.e. a sense of control) in our lives has to be the 'holy grail' for public
health to be truly effective.

And the troubling question is, are they caused by "lifestyle" i.e. "it's
your fault and if you'll only follow my advice you can change it" or by
the
"social setting" which makes the ability to resolve things radically
different for cultural communities and by social grade.

hypertension, obesity, lower back pain, osteoporosis, and even cancer,
