Several operations worldwide are trying to do exactly what your
purport to do; they are critically reviewed in a very thorough and
useful manner in a document prepared by Jackson et al. last year for
the Canadian consortium of Health Promotion research. The following
link provides their analysis as well as links to most of the major
evidence seaking initiatives in health promotion across the world
like the Cochrane Collaboration, etc.
