I agree with Monica on Baum.

There is another aussie gem called "Theory in a Nutshell" by Don Nutbeam and
Elizabeth Harris (McGraw-Hill 1999). It dioes not cover social capital and
community building very well but all earlier theory is well documented.

The Evidence of Health Promotion Effectiveness (IUHPE 2000) is good

Finally the WHO "The Social Determinants of Health" is great for instigating
discussions with students and health professionals alike.
