Hello:
We are preparing a national survey on characteristics of health promotion
in primary care in Canada.
Our eventual aim is to build from the identified characteristics to
validated, measurable indicators to concrete standards for specific aspects
of health promotion in primary care settings.

We are trying to identify a comprehensive list of "community health
centres" across the country. We recognize that it is hard to definitively
identify what constitutes a  "community health centre" .Our group is
primarily interested in surveying community-based entities with a high
likelihood of engaging in health promotion, broadly defined.

We would appreciate any suggestions, potential contact information,
directories, e-lists, etc. on how we can identify community health centres
or other relevant organizations in your province/territory. We will be
pleased to share our results with any, and all, interested parties.
