The Canadian Public Health Association is participating on the advisory
committee for a project of the Active Living Alliance for Canadians with a
Disability (ALACD). ALACD, in collaboration with advisory committee members,
is producing guidelines and resources to address the Prevention, Detection
and Management of Diabetes for Canadians with Disabilities. 

We are seeking your input to prepare useful resources for practitioners that
provide services for Canadians with disabilities, including those working in
public health and a range of other disciplines and settings. The following
survey is designed to identify the nature of your agency, community setting,
your clients with disabilities, the impact of diabetes on your service
provision, the type of resources you would find beneficial and the format in
which the information is best made available for you.

L’Association canadienne de santé publique participe aux travaux du comité
consultatif pour un projet de l’Alliance de vie active pour les
Canadiens/Canadiennes ayant un handicap (l’Alliance). L’Alliance, en
collaboration avec les membres du comité, doit produire des lignes
directrices et des ressources touchant la prévention, la détection et la
gestion du diabète chez les Canadiens et Canadiennes ayant un handicap. 

Nous sollicitons donc vos commentaires afin de mieux préparer des ressources
qui seront utiles aux praticiens fournissant des services aux Canadiens et
Canadiennes ayant un handicap, y compris ceux et celles qui travaillent en
santé publique et un éventail d’autres disciplines et contextes. Le sondage
qui suit vise à identifier votre agence, votre contexte communautaire, vos
clients ayant des handicaps, l’impact du diabète sur la prestation de vos
services, le genre de ressources qui vous seraient utiles et le format dans
lequel vous préférez recevoir de l’information.
