As part of our commitment to client and partner satisfaction and outcome
measurement, the Ontario Prevention Clearinghouse (OPC), including the
Health Promotion Resource Centre and Best Start Maternal Newborn and Early
Child Development Resource Centre, are conducting a feedback survey. Between
December 1st and December 15 an evaluation team, under the auspices of VIRGO
Planning and Evaluation, will be contacting a random sample of people who
have been involved with OPC/Best Start as users of services and in
colalboration on committees and projects over the past year.

You may be called upon in the next week to complete a brief (5-10 minute)
and confidential "survey" by phone or by email.  Your participation in this
evaluation is very much appreciated.  A summary of the findings will be
available early in the new year. Thanks for your help!
