I have given a lot of thought to this -- building upon critiques that Ron
Labonte first presented in 1979!! And that of British workers such as Sarah
Nettleton, Davey Smith, Marmot and MANY others.

My website has my publications and presentations related to this question.
Specifically the heart health, diabetes, and critiques of public health
approaches papers.  Other references from others would be of value!
