Thanks for this helpful summary.
There is a related issue which you touch on but which I think it would
be helpful to develop further - namely the intergenerational
transmission of behaviours related to the rearing of children that are
not conducive to healthy outcomes in them throughout their life course,
and the continuing exposure of young children to environmental
experiences that are "biologically embedded" and affect subsequent
responses to stress and impact upon immune functioning. The Canadians
have been at the forefront of this exposition as illustrated by Keating
and Hertzman's excellent overview in "Developmental Health and the
Wealth of Nations" which you reference.
