55th Annual Ontario Public Health Association (OPHA) Conference
PUBLIC HEALTH: THE BEST HEALTH INVESTMENT

CONFERENCE DATE: November 23-24, 2004
LOCATION: Marriott Toronto Eaton Centre

Abstracts can be submitted for one of three presentation formats:

1. Oral Presentation
2. Interactive Workshops
3. Posters

Specifications for Abstract Submissions are as follows:

* Abstract submission format: Not to exceed 250 words and to include a
20-word abstract brief. Information to include title, authors names and
titles, lead presenter and contact information, relevant theme (see below),
format preference and alternate presentation format

* Abstract submission method: Electronic submission is a requirement using
the form available on website at www.opha.on.ca/conference or by contacting
Marlene Mirza at [log in to unmask] [905-546-2424 ext. 1578] if you cannot
access the website.
