Provincial Needs Assessment Questionnaire for Health Promotion Services and Supports

Dear Colleague,

We are contacting you because you have used the services or supports of at least
one of the organizations that make up the Ontario Health Promotion Resource
System (OHPRS). This may have included, for example, a training workshop,
curriculum support, consultation, newsletter, or Listserv discussion. To better
acquaint you with OHPRS, you may wish to visit the web site at
http://www.ohprs.ca. We are looking for your input that will assist the
participating organizations in more effectively supporting health promotion
activities both individually and collectively.

This survey is about services and support for your work as a "health promoter".
We define "health promoters" quite broadly as a diverse group of professionals
and volunteers working in such areas as public health, education, prevention,
community development/capacity building, self-help/mutual aid, environmental
issues and health service delivery. Their work may focus on any or all of the
broad determinants of health, including education/literacy, income, social
support networks, employment/working conditions, social and physical
environments, personal health practices and coping skills, healthy child
development, health services, gender and culture. Health promotion work may be
paid or voluntary.

Your individual answers to the survey questions are anonymous. Only grouped
answers to the quantitative questions will be reported, as well as selected, but
anonymous, quotes to highlight key themes in the open-ended responses. In the
Spring 2005 a report summarizing the findings will be available on the OHPRS
website.

To complete the survey on-line you may access the questionnaire in English at

We would appreciate receiving your completed questionnaire by October 25, 2004.

In appreciation of the time you have given us you may elect to participate in a
FREE DRAW for registration and up to $500 travel support to a health
promotion-related conference in Ontario sponsored by one of the OHPRS
organizations. Instructions to participate are included in the questionnaire. Do
not hesitate to pass on the contact information and Internet link to the survey
to colleagues working in health promotion and whom you feel would be interested
in participating. They may participate in the needs assessment whether or not
they have used any services or supports from an OHPRS organization.

The survey questionnaire consists of the CORE questions (estimated completion
time is 20-25 minutes). The questions cover barriers to accessing information
and technical support for health promotion work, including language and
multicultural issues; self-help/mutual aid strategies; preferred approaches to
meeting your education and training needs; and French language services and
supports. These issues are relevant to a wide cross-section of health promoters.

In addition, there are SUPPLEMENTARY modules to the questionnaire that will be
of interest to particular health promoters, depending on the type of activities
in which you are engaged or which are of general interest to you. On average the
completion time for each module is about 5-15 minutes.

    * Maternal and Infant Health
    * Nutrition
    * Physical Activity
    * School-based Health Promotion
    * Tobacco Control
