MessageYour assistance in passing this request on throughout your networks associated with Chronic Disease Prevention is much appreciated.  

On behalf of the Centre for Chronic Disease Prevention and Control at PHAC, and the CDP Best Practices Steering Committee, I am requesting a few minutes of your time to complete this on-line survey found at  http://www.media-doc.com/clients/phac/ .  The survey is requesting input to the development of a Web-based portal and database through which practitioners, researchers and policy-makers would access "best practices" in chronic disease prevention and control.  The survey will be available until October 11th.
