Physically Active for Community Health: The Needs of Leaders

As a person involved in physical activity, sport, recreation, and fitness
you are invited to participate in a consultation to guide curriculum
development for the Atlantic Summer Institute on Healthy and Safe
Communities by completing a survey described below.  

* NOTE: You do not need to be from Atlantic Canada to participate - all
opinions are welcome!

The purpose of this consultation is to confirm what is needed in a learning
program for community leaders to address physical activity in the context of
community conditions and the social determinants of health. 

In the survey questionnaire, you will be asked for your opinion on a number
of statements and also asked for your own thoughts on what is required in a
curriculum. The statements in this survey are the result of a consultation
that took place in Prince Edward Island earlier this year.  We are seeking
your input to validate that the opinions are shared by the wider community
in Atlantic Canada. Findings of the consultation will be used to make
recommendations to the ASI Board to develop curriculum. 

The Atlantic Summer Institute on Healthy and Safe Communities has engaged
the services of The Quaich Inc. (www.thequaich.pe.ca), a Charlottetown based
consulting company to conduct the survey.

Please note: The survey will close on September 9, 2009.

What you should know about the survey
 Your participation is voluntary. 
 The survey should take approximately 20 minutes. 
 You can respond only to the questions you feel comfortable answering. 
 The information you share will be kept confidential. 
 The information you share will be stored at The Quaich Inc. in a secure
location and will be destroyed when the project is complete. 
 Only the staff of The Quaich Inc. will have access to your information.
