Note that the link goes to a *large* PDF, which could be a problem for
people with a slow connection and/or paying for downloading time and
storage. It is a very interesting publication worth reading. Consider the
difference between 'best practices' in North America, and 'good practices'
in Europe, and how 'social inclusion' and 'health inequalities' may also
differ.
