Choices. This assessment tool is comprehensive and may guide you in
developing this survey. It may also in itself meet your needs. Consult

Subject: Secondary School Survey
Hi,
As part of our Heart Health School Working Group in Hastings and Prince
Edward Counties, our working group is going to conduct a survey with
secondary school students and teachers on the three risk factors: physical
activity, tobacco use, and healthy eating. I am inquiring if other groups
have conducted this type of survey before addressing these risk factors with
this population. Also, looking for best practices or previous surveys that
have been used with secondary school students that address these three risk
factors.
