Many thanks,

Call for Submissions for 2006 Day Planner

Building on the success of last year's calendar, Horizons Community Development Associates is now inviting submissions for Celebrating Community in Atlantic Canada 2006! We would like to include pictures, stories, biographies and artwork that represent the diversity and scope of health promotion work in this region, whether or not it is called 'health promotion'.

We want to support health promotion organizations in Atlantic Canada. This year we will be drawing from among the non-profit and community organizations that send in their submissions by May 31st, 2005. The winning entry will receive 25% of the proceeds from selling the Day Planners.

Ideas for Submissions:

· Write a story (maximum 200 words in Word or Word Perfect format) about the health promotion work of a person, group, community, or organization in Atlantic Canada that you think merits recognition and tell us why

· Send in a photograph (in jpg format, high resolution, minimum 300 dots per inch) of a person, group, organization, or community whose health promotion work merits recognition and tell us why

· Send in a poem or quotation (with exact reference information, up to 200 words in Word or Word Perfect format) that celebrates health promotion to you and tell us why

· Send in an original poem, drawing or story (200 words in Word or Word Perfect format) that celebrates health promotion for you

· Send information about an important health promotion initiative you would like to have included in the Planner (no more than 75 words, Word or Word Perfect format). This initiative could be small and local, big and provincial, or anything in between!

· Send the date of your special event - we'll list it in the calendar.

· Another idea? Please contact us - we're open to hearing it!

The deadline for submissions is May 31st, 2005. The Planners will be ready by early October.

Please complete the form below and include it with your submission.
