Job Description - Knowledge Transfer Coordinator
CACHCA and AOHC 
The Canadian Alliance of Community Health Centre Associations (CACHCA) was established in 1995 to provide support to Canada's provincially based community health centre organizations (CHCs) and to represent the interests of those organizations at the national level.



The Association of Ontario Health Centres (AOHC) is the not-for-profit organization of community health centres (CHCs), aboriginal health access centres (AHACs), and community health service organizations (CHSO). Our member centres throughout Ontario provide community-based primary health care, health promotion and illness prevention services to tens of thousands of Ontarians in over 70 communities.



CACHCA and AOHC have received funds from Health Canada's Primary Health Care Transition Fund to implement two National projects.  



Project: Disseminating Best Practices in Interdisciplinary Teamwork
Supporting interdisciplinary collaboration is one key area in primary health care reform.  Community Health Centres have over 30 years of experience in this type of work.  CHCs deliver work with individuals, groups, and the larger community using teams that typically include physicians, nurses, nurse practitioners, health promoters, social workers, community developers, and others.  The purpose of the project is to tap into the experience of CHCs to strengthen the capacity of this sector to do effective interdisciplinary work, and to share this experience with other primary health care models across Canada.



The objectives of the project are:

1.      To develop best practice guidelines for effective functioning of interdisciplinary teams in CHCs;  

2.      To develop standardized tools for evaluating and assessing the effectiveness of interdisciplinary teams in CHCs; and

  3.. To develop and disseminate educational tools for providers that support the development of the skills needed to work effectively in interdisciplinary primary health care teams. 
Role of Knowledge Transfer Coordinator:
The Knowledge Transfer Coordinator is the primary person responsible for designing and coordinating the development of resource materials and the dissemination of these materials (including designing and coordinating a series of workshops with CHCs and other primary health care providers across Canada).



The Knowledge Transfer Coordinator will work under the supervision of the Project Manager, and will work closely with a Steering Committee and with consultants who will develop the content of the resources and training workshops.

Responsibilities:
  a.. Consult with CHCs and other primary health care providers regarding the design of the resources and workshops. 
  b.. Review relevant literature and other resources related to interdisciplinary teamwork. 
  c.. Work with the consultants and the Steering Committee to coordinate the design and production of the resource materials and workshops. 
  d.. Identify consultants and/or resource people who can serve as regional experts for CHCs. 
  e.. Organize the training session for regional resource people. 
  f.. Organize regional workshops for CHCs and people working in other primary health care models. 
  g.. Ensure that resource materials are organized to be mounted on a website. 
  h.. Organize the broader dissemination of resources produced through this project. 
  i.. Keep the Steering Committee, AOHC and CACHCA well informed about the progress of the project. 


Extent of contract:
  a.. Salary to maximum of $70,000 per year, plus generous benefit package  
  b.. Full-time, from September 2005 to September 2006 
Qualifications:
  a.. Masters in Health related discipline or equivalent combination of educational and work experience  
  b.. Knowledge of primary health care 
  c.. Experience in resource development and dissemination 
  d.. Experience in design and facilitation of workshops 
  e.. Bilingualism a strong asset 
  f.. Strong organizational and coordination skills 
  g.. Knowledge of CHCs an asset
