If someone hasn't already suggested it, I would recommend that you contact Saskatchewan Health ([log in to unmask]) and ask for a copy of "A Population Health Promotion Framework for Saskatchewan Health Districts." It's an excellent resource, incorporating and expanding on the Hamilton/Bhatti model.

Subject: Population health framework
We are looking for a population health framework for both planning and
integration of population health perspectives and strategies across a
regional health system (i.e. the whole ball of wax, from prevention and
primary care to tertiary and long-term care). In particular, we are looking
for a framework that combines population health and health promotion.
We have the Bhatti and Hamilton Population Health Promotion model but are
also interested in other models.
Any ideas?
