There is a good text book we used in the course I recently took at Memorial University entitled "Workforce Education The Basics"  Author is Kenneth C. Gray and Edwin L. Herr ISBN  0-205-19834-1  This text does not address education as a determinant of health per say however it gives a good overview of how education or lack thereof has affected the overall economic "health" of our nation.
