Hello Bill:

You might check out the "Intervention Wheel" as a population-based
practice model, previously known as the Public Health Intervention
Model, which was introduced by public health nurses in the Minnesota
Department of Health in 1998. How it has evolved since then is described
in these articles:

Thank you, Dennis. This appears to be a research template more than a 
model (as I read it). I am inquiring into models that have feedback 
loops, that illustrate dynamic behavior over time.
