Hi Peter,
I really like this chapter by Bruce Link and Jo Phelan that recently
appeared in an edited volume published by the Robert Wood Johnson
Foundation. I find it works well with non-researcher audiences and has
some ideas for policy. Here's the link to the book's table of contents
where you can download the pdf of their chapter "Fundamental Sources of
Health Inequalities"...
