They are not included in here, because most of these indicators are
derived from the National Vital Statistics Records (birth and death
records) to represent trend over time (in the most simple sense). 
Therefore, they don't include income level.  However, we do collect
information regarding income levels through census or surveys.  Analyses
by income level will need data linkages between the vital statistics
system and census data or survey.  Sometimes could be a major
undertaking.  I believe it's being done by academic researchers and are
usually published in peered reviewed journal articles.
