Seems pretty obvious to me. If you don't have access to regular and ongoing health care because your are poor, less informed about the availability of services and need for preventive care, you are more likely to delay detection until you feel sick. So when you are finally diagnosed, you are already at a more advanced stage. To make matters worse, you don't have money to pay for treatment or you are already at advanced stage that treatment will less likely to help.
