Dear SDOH colleagues,

Moving Upstream: Working Together to Create Healthier Communities, a
policy forum on social determinants of health held on November 13, 2006
in Bloomington, Minn., is now available on DVD at no charge from the Blue
Cross and Blue Shield of Minnesota Foundation.

The DVD features presentations by local and national experts and
discussion among leaders representing Minnesota's health, environmental,
early childhood, community development, housing, business and policy
sectors. Also included are two televised programs based on the forum,
coproduced with Twin Cities Public Television.

Knowledge can be a tool for change. By sharing research, stories, models,
tools and policy opportunities, the Foundation hopes to broaden awareness
of the root causes of health disparities and how to take action. You can
order a copy of the DVD online at www.bluecrossmn.com/foundation, by
e-mail at [log in to unmask], or call (651) 662-3950 or toll free
1-866-812-1593.
