This is the website for an Ontario, Canada effort to improve access to
health care in regions where at present substantial numbers of
residents do not have access to their own family physician. As a
strategy it builds on the existing resources of the community, with
additional resources provided for the collaborative effort.

The Prince Edward Family Health Team, covering Prince Edward County
(Ontario) has launched its website, designed to inform and involve
the citizens of Prince Edward County in this collaborative effort to
improve access to medical care for the County's 24,000 residents.
Prince Edward County is a virtual rural island county in Lake
Ontario, linked to the mainland by bridges and a ferry service.

In April of 2005 the Ontario government launched a program of urban
and rural Family Health Teams as a core component of an Otario health
care initiative to provide better access to doctors and nurses and
reduce wait times for treatment.

The 150 local Family Health Teams across the Province are
collaborations involving individual local health care providers and
agencies. The teams are to improve access to primary care for more
than 2.5 million Ontarians in 112 communities. The teams include an
interdisciplinary team of physicians and other providers such as
nurse practitioners, nurses, social workers and dieticians, with the
goals of working together to see more patients and to keep them
healthy.
