Greetings:
I am wondering if anyone on this listserv involved in chronic disease
prevention and management knows of some good international 'best/promising
practice' examples of INTEGRATED, SYSTEMS APPROACHES TO ADDRESS CHRONIC
DISEASE RISK FACTORS vs disease-specific management approaches (specifically
looking for models and examples that address populations of clients/patients
with high blood sugar, high cholesterol, high blood pressure,
obesity/overweight)... I had 'heard' that in Scandinavia there is some kind
of a risk factor "boot camp" that patients with elevated risk can attend,
but I don't know if this is true??!!
I am hoping to find some international examples of integrated risk factor
approaches to prevention and management of chronic diseases that hopefully
use/build on the MacColl Chronic Care Model, etc...
Any ideas/suggestions/contacts?
