The Stanford Chronic Disease Self-Management Program is internationally
recognized as a key component in health renewal to support self-care,
ultimately resulting in improved health outcomes and decreased acute service
utilization for persons with chronic conditions. Patient self management is
a critical untapped resource in healthcare improvement. The Alberta
initiative will support and capitalize on the strengths of self management
by providing a supportive context for positive self-management through
exemplary leadership and modeling of leaders and trainers.
The Provincial Stanford Chronic Conditions Self-Management Program in
Alberta is primarily funded by Alberta Health & Wellness. The target
audiences for this program are Primary Care Networks and Regional Health
Authorities. This program utilizes an train the trainer model’ all sessions
are co-facilitated by two trained leaders. The leaders are health care
professionals or volunteers who follow a standardized scripted course
outline. The purpose of this leader training program is to:
* build resources & expertise in all Primary Care Networks and RHAs
* ensure comprehensive province-wide support of chronic conditions
self-management programs
This program complements various chronic conditions management programs that
Primary Care Networks and regional health authorities are currently
developing. The purpose of a Stanford workshop is:
* Help people positively manage their life with chronic conditions
* Increase health self-management skills and self-confidence
* Enhance the skills required to problem solve and make decisions
related to living a healthy life with a chronic condition

Health In Action is an on-line resource to access information on provincial
health promotion and injury prevention contacts, programs, and research. We
also enable a select number of health promotion or injury prevention project
to have a web presence that they would be unable to have their own. One of
these projects is the Provincial Stanford Chronic Conditions Self-Management
