Scott, it is indeed expensive. Cambridge University Press markets these primarily to libraries. I hope you will find a library you can see it from. And I hope it proves helpful to you. The essays on various topics are by many noted Twain scholars. It was a pleasure to collect them and edit the volume.

I've only now just become aware of this book by John Bird. I have not
found an in depth discussion of its content but from what I've seen it
would contribute greatly to my own work with Twain's Geography.
Unfortunately the cost for acquiring a copy is far beyond the means of
this minimum wage worker. Should anyone have a spare copy and would not
mind sending it out on loan, or if some hidden on-line version be
available I'd very much like to see it.
