I am part of a research team right now looking into program planning and evaluation practices. I am writing to find out from you about the practices in your workplace/health unit.

 1.
Do you have a corporate or departmental policy/procedure/mandate to do program planning and evaluation?
 2.
Is program planning and evaluation being "done" constistently from team to team, department to department?
 3.
Do you have standardized templates and forms that you use? Are they mandatory?
 4.
Do you follow the THCU planning model? If so, do you follow it exactly or have you come up with your own version?
 5.
Are you using THCU templates? If so, which ones do you use? Have you modified THCU templates?
 6.
If you are using any other models and/or templates, what are you using & where did they come from?
 7.
In your department and/or team, who is responsible for "doing" program planning & evaluation?
 8.
How are new staff made aware of the program planning & evaluation practices in your workplace?
 9.
How are staff trained on the program planning & evaluation practices in your workplace?
 10.
Is this training a one time event, annual, as needed, etc?
