Request for Applications: Ottawa Model for Smoking Cessation 

With funding from the Heart and Stroke Foundation of Ontario and Pfizer
Canada, the University of Ottawa Heart Institute is making
implementation start-up grants of $12,500 available to hospitals not
already engaged in the Ottawa Model for Smoking Cessation. The Request
for Applications form, application criteria and background on the model
can be downloaded by visiting www.ccpnetwork.ca
<http://www.ccpnetwork.ca>  and clicking on 'Champlain Hospital-Based
Smoking Cessation Network'. Completed RFAs are due by September 30th,
2008.
