Acting on the social determinants of health (SDOH) - a Twitter Chat

Join us Tuesday, September 24 from 3-4pm for #HealthPromoChat <http://en.healthnexus.ca/health_promo_chat>, hosted by Health Promotion Ontario (HPO) conference coordinator Tanya Beattie.

In anticipation of the Sep 26th HPO conference 'Think Big and Let's Get Going: Applying SDOH<http://www.hpontario.com/> '  we'll be talking about a SDOH continuum of activities that move upstream. Share your current work and challenges in education and awareness, programs, initiatives, resources, policies and inter-sectoral action. Let's keep the conversation, and action, going!

Questions to explore:
How are you or your organization acting on the social determinants of health?
What enables moving from awareness to advocacy?
What does 'Moving or Working Upstream' mean to you? Do you have stories of making that move upstream in your work?
How do we genuinely connect with other sectors who are active in working on determinants, such as housing, poverty, women's health, recreation, municipal planning and more?
