Hi Charlene,

Unfortunately I have not been able to find much literature on this matter
myself.  However, I have started a Facebook and Twitter page dedicated to
increasing self-advocacy for health decisions through information, education
and empowerment.  If you find anything that you feel that should be readily
acessed by the public please feel free to forward me some information and I
