Hello,
Here is a link that could be useful:
http://www.healthbegins.org/
and then enter the section: Get a free copy of the 'Upstream Risk Screening Tool"
They will send you a tool that is nicely done.
Hope this helps,
Lyanne

Subject: [BULK] [SDOH] SDOH template
Importance: Low
Hello,
I am a social worker employed in a hospital setting. In my report on each patient I work with, I include information on the patient’s financial situation, housing, education, and resources. I usually place this info under the heading of SDOH, but am wondering if there is a more organized way to do this, one that possibly provides a revealing snapshot of a patient’s situation from a SDOH perspective. Do you know of a format or template that might be of use in this situation?
Thanks so much,
Patricia Swick
